Briggs Form 1265 is uniquely designed to allow you to remove copies without taking the actual form out of the patients chart. After the physician writes an order, Parts 2 and 3 are removed as a set, then separated for distribution to Nursing and Pharmacy. Pharmacy and Nursing copies are cut below the stub allowing quick removal while the top sheet stays in the chart. A backing sheet (Part 4) holds all parts together. Available with 5-hole top punched (1265P) or non-punched (1265).
