Save Time! Briggs has made insurance claim filing easier and faster. No need to type envelopes or print mailing labels. This envelope has a window where the mailing address shows through. In addition, the self-sealing feature makes for fast, clean closure. Here is how it works:
1) Type or print the name and address of your intermediary in the upper right-hand corner of your Briggs HCFA 1500. NOTE: Be sure your HCFA 1500s have the space for an address,
2) Fold the HCFA 1500 at the marks indicated on the left and right sides of the form.
3) Insert the form so that the intermediarys address shows through the window.
4) Fold over the self-sealing flaps to close the envelope.
